Default Erythema Migrans

Let me preface this by saying I will go to the doctor, but in meantime I'm looking to get some opinions. This rash appeared a little less than a month ago, after working in the yard. It started off about the size of a quarter and has gotten progressively bigger. I have tried antifungal, antibacterial and steroid creams. Nothing has helped. Past few days I've had a headache with a lot of pressure behind it. I've been dizzy and nauseated on/off along with heart palpitations, weakness in hands and low grade fever.
My rash has a bit of a raised border. Ringworm was pretty much ruled out. Has anyone ever seen erythema migrans that looks like this?
